Forest Family Care joins Ballad Health Medical Associates

Ballad Health is proud to announce that Forest Family Care has joined Ballad Health Medical Associates, expanding the system’s mission to honor those we serve by delivering the best possible care throughout our region.

As part of this transition, the practice has retained its name and is known as Ballad Health Medical Associates – Forest Family Care. It is located at 1785 W. Lee Highway, Wytheville.

This collaboration ensures continued, high-quality care for the community while providing patients seamless access to Ballad Health’s extensive network of specialists, facilities and services.

Patients of Forest Family Care see Susan Moore, FNP-C; Jan Rasnake, FNP-C; Misty J. Colley, FNP-BC; and Susan Price FNP-BC.
