Scheduling an appointment

Once your doctor provides a referral, you or your provider's office can contact our clinic directly to schedule your first visit.

After your first visit is scheduled, you can use the Ballad Health app to view and manage your healthcare appointments, access visit details and keep your schedule organized in one convenient place.

Preparing for your first visit

Please arrive about 15 minutes prior to your scheduled visit to complete new patient paperwork. You should also bring your physician order for therapy, insurance card and a photo ID.  

We also recommend you wear comfortable clothing that allows for easy movement. You can expect to begin treatment on the same day as your initial visit and evaluation.
