How to get started with outpatient rehab or physical therapy?

Do I need a referral?

Many patients begin outpatient therapy with a doctor’s referral, though insurance requirements may vary.

We recommend that you speak with your doctor or provider about any conditions you have that may cause pain or restrict your activities.

Scheduling an appointment

Once your doctor provides a referral, you or your provider's office can contact our clinic directly to schedule your first visit.

You can also use the Ballad Health app to view and manage your healthcare appointments, access visit details and keep your schedule organized in one convenient place.

Preparing for your first visit

Please arrive about 15 minutes prior to your scheduled visit to complete new patient paperwork. You should also bring your physician order for therapy, insurance card and a photo ID.

We also recommend you wear comfortable clothing that allows for easy movement. You can expect to begin treatment on the same day as your initial visit and evaluation.

Frequently asked questions about outpatient rehab and our clinic

Outpatient rehabilitation includes therapy services you receive at a clinic while continuing to live at home. Appointments are scheduled, and you’ll return home the same day like any other typical doctor’s visit.

Physical therapy focuses on movement, strength, balance and comfort. Occupational therapy helps support everyday activities like getting dressed, bathing, cooking and working.

A physical therapist helps people move better, feel stronger and manage pain. Our therapists create personalized treatment plans with exercises, hands-on care and education to help you improve your strength, balance and flexibility. Physical therapists also help patients who are recovering after an injury or surgery.

Speech therapy supports things like communication, memory, thinking skills and swallowing. It’s used to treat a variety of conditions – from stroke to stuttering help.

A wheelchair evaluation helps identify mobility equipment that fits your needs and supports safety and comfort. Evaluations are completed by our ATP-certified specialists.

Some exercises may feel challenging, but therapy should not cause sharp pain. Your therapist will work with you at a safe pace that aligns with your needs.

Some patients feel better after a few visits, while others feel improvements gradually. Your therapist will track your progress and adjust your plan as needed.

Patient parking is available directly in front of the building. Please use the entrance under the blue awning.

We recommend wearing comfortable clothing and supportive shoes so you can move easily.
